The health services provided in the Universal Health Solutions™ program are NOT insurance and are not intended to replace insurance. They are reduced Fee For Service programs that provide discounts at certain providers. Members must pay the entire reduced cost for the health services performed at the time service is rendered unless other arrangements are made with the provider's office. Universal Health Solutions™ does not pay providers.

This program is not protected by any Life and Health Guarantee Association. Universal Health Solutions™ is not considered a program for purposes of creditable coverage. A certificate of creditable coverage will NOT be issued upon termination of a discount program.

This Plan is not available to residents of Florida, Oklahoma, Connecticut, Montana, Nevada, Indiana, South Carolina, Kansas, Ohio, Nebraska, North Dakota, Oregon, Missouri, Maryland, Texas, Tennessee, West Virginia, New Hampshire, Louisiana, South Dakota, Washington, or where prohibited by law.
